Thank you for opening a credit account with OpticalLab.ca. All accounts are subject to the following terms and conditions:

Orders are shipped on Net 30 terms unless otherwise agreed. Invoices paid within thirty (30) days are not subject to finance or service charges.

Invoices not paid within 30 days may incur interest charges of 1.5% per month (18% per year) as outlined in the account agreement.

Credit accounts are non-transferable. The account holder agrees to notify OpticalLens.ca of any change in ownership, legal structure, or business location within ten (10) business days.

The applicant is responsible for ensuring all prescriptions, measurements, and order details submitted are complete and accurate. OpticalLens.ca is not responsible for errors resulting from incorrect or incomplete information provided by the account.

By submitting a credit application, the applicant authorizes OpticalLens.ca to obtain and review credit information related to the business and its principals for the purpose of establishing and maintaining credit terms.

OpticalLens.ca reserves the right to limit credit availability, place accounts on hold or C.O.D. status, refuse or cancel orders where necessary, and suspend or terminate credit privileges at its discretion. Accounts in default may be placed on hold until balances are resolved.

Upon account approval, OpticalLens.ca may request payment method details, including banking information or credit card authorization, in order to activate billing and ensure uninterrupted service.
